I hit my head at work the other day and needed to fill out an incident report... the questions ran something like this...

What is the nature of the injury?

Cut above right eye. Bleeding. Did not require stitches. Concussion.

Where did the injury occur?

Ground floor toilet.

How long have you been preforming the task which you were undertaking at the time of the injury?

Umm... 30 years and 2 months...?
